Step-by-step guide for adults: preparation, correct injection, dosing and storage. Once a week, subcutaneously.
What is Mounjaro®?
Mounjaro® is a tirzepatide-based medication from Eli Lilly, available in KwikPen® pre-filled injection pens. Administered subcutaneously once a week.
How does Mounjaro® work?
Mounjaro® mimics the action of two gut hormones (GIP and GLP-1) simultaneously, which sets it apart from other drugs in its class.
Dosing schedule
Always start with 2.5 mg — regardless of the goal (weight loss or diabetes). Dose increases should happen no sooner than every 4 weeks.
Step-by-step instructions
Check the product name, dose and expiry date on the label. The solution should be clear, colourless or slightly yellow.
Wipe the red inner seal with a swab. Always use a new needle for each injection — to prevent infection.
Press the pen vertically onto the needle and twist until firmly attached.
Remove the outer cap and keep it. Remove the inner cap and discard it.
Slowly turn the dose button until you hear 2 clicks and the air-shot line appears in the window.
Gently tap the cartridge. Press the button all the way down and count to 5. The symbol ⑩ should appear in the window. A small drop at the tip means the air has been removed successfully.
Three possible zones: abdomen (at least 5 cm from the navel), thigh or upper arm. Rotate injection sites each week.
Turn until it stops and the symbol ① appears in the window.
Insert the needle into the skin. Press the button all the way and slowly count to 5. The symbol ⑧ should appear in the window — the full dose has been delivered. Remove the needle.
Replace the outer cap. Unscrew the needle and discard it in a sharps container. Replace the pen cap.

Side effects
The most common are nausea and diarrhoea — mainly at the start or when increasing the dose. In most patients they resolve on their own.

Once a week, subcutaneously, on any convenient day. It is best to inject on the same day each week — for example every Friday or Sunday.
If fewer than 4 days have passed — inject as soon as possible. If more than 4 days — skip it and wait for your next scheduled dose. Never inject a double dose.
Before opening: 2–8°C in the refrigerator, do not freeze. After opening: below 30°C, no more than 30 days. Keep away from light and heat.

Only compatible disposable pen needles. Never reuse a needle — it increases the risk of infection and blockage.
Most people feel only minor discomfort. To minimise it: let the pen warm to room temperature for 30 minutes, and insert the needle at a straight angle.
Always 2.5 mg for the first 4 weeks — regardless of the goal (weight loss or diabetes). This is the adaptation period. After that, your doctor may increase to 5 mg and beyond.

Eat small portions, avoid fatty foods and drink enough water. Try injecting in the evening before bed — nausea tends to be less noticeable in the morning. Side effects usually resolve after the adaptation period.
